Please bring the following records if available with you to your first visit:

• Copies of all office records from the referring Neurologist.
• Copies of all office records from any other doctor currently involved in your medical care.
• Reports of all blood work.
• Reports from CSF studies (spinal tap results).
• Films or CD ROM of all MRI scans of the brain and spinal cord (not reports).
• Reports from other diagnostic tests (e.g. evoked potentials).

*Note: We request that you personally provide all of the above items even if your doctor's office states that they will fax / mail them.*
