Please bring the following records if available with you for your first visit:

Copies of all office records from the referring Neurologist.
Copies of all office records from any other doctor currently involved in your medical care.
Reports on all blood work.
Reports from CSF studies (spinal tap results).
Films or CD ROM of all MRI scans of the brain and spinal cord (not reports).
Reports from other diagnostic tests (e.g. evoked potentials).

*Note: We request that you personally provide all the above items even if your doctor's office states that they will fax / mail them. *

Take I-94 west to southbound I-75. Stay in the right lane. Once you are on I-75 south, take the Mack Avenue exit. Turn right or west onto Mack Avenue. Take the I-94 Freeway east to southbound I-75. Stay in the right lane once you are on I-75 south and take the Mack Avenue exit.
