Application Information

Thank you for your interest in the Clinical Neurophysiology Fellowship Training Program at Wayne State University/ Detroit Medical Center.  As we offer several tracks of fellowship training, please be specific when you contact us and let us know which fellowship you are interested in.

  • EMG/ Neuromuscular Track
  • Combined EEG/ EMG Track
  • Neuromuscular Track
Please download the application form, complete it and return it along with the pertinent documents listed below. The applicant’s fellowship period, the degree to which previous training can be applied toward eligibility for Board Certification and stipend will be determined by the program director. This decision is based on the rules and regulations of the American Board of Psychiatry and Neurology, Inc. and Hospital Residency Training Program Guidelines.
 
Documents required along with a completed application form:
  1. Curriculum Vitae
  2. Personal Statement: Please provide a personal statement describing your career goals, the reasoning behind your choice of the field and the fellowship program
  3. Confirmation of Residency training and Board eligibility/certification: A letter confirming the anticipated date of the successful completion of residency program is required. Once you complete the residency program provide us a copy of the completion of the training certificate
  4. Medical School Transcript of credits: An official transcript will be required of all serious applicants
  5. Letters of Recommendation: Three letters should be sent directly to the Program Director. These letters should attest to your personal qualification and to your scholastic and clinical abilities. One letter should be sent by the Clinical Director of your residency training program, and one letter by a staff or faculty member currently practicing the specialty desired, if possible
 
Information for International Medical Graduates (IMG): If you are an IMG, please enclose a photocopy of
  • ECFMG certificate
  • USMLE scores
  • Copy of Medical School diploma (provide a certified translation of the diploma if it is not in English)
A personal interview is highly desirable for all qualified applicants, and will be coordinated through the Departmental Secretary upon completion of the application and receipt of the recommendation letters.

If selected for the fellowship, we will send a letter of offer. Please sign and return the letter to us. The actual appointment and contract are handled by the Graduate Medical Education department. You will be sent all the necessary documents to complete the process. It will not be possible to place you on the payroll until all of the required documents are received and found to be in order. In addition, you need to apply to the appropriate Board of Registration for Michigan Medical License and Drug registration. You have to obtain either permanent or educational medical license in State of Michigan prior to start of your fellowship. We can send you information regarding this upon request.
